The Southeast Iowa Complex Care Network recently announced that it was awarded a grant from Iowa’s Centers of Excellence Program. The grant totals $250,000 per year for three years, and will help enhance access to specialty care in rural communities.
The Southeast Iowa Complex Care Network is a groundbreaking collaboration which includes Jefferson County Health Center, Van Buren County Hospital, Davis County Hospital and Clinics, and other dedicated local partners. Their collaborative care model is centered on three fundamental principles: improving the patient experience of care, improving the health of populations, and reducing the per capita cost of healthcare.
With this funding from Iowa’s Centers of Excellence, the Southeast Iowa Complex Care Network can more effectively address the unique healthcare needs of the elderly, including limited mobility and transportation challenges. The network’s ultimate vision is to ensure that specialized, high-quality, and preventative services are readily accessible to geriatric residents in Van Buren, Davis, Jefferson, and Lee counties, while increasing their utilization.
